Notes-History Form

I need to be able to Add a Second Notes-History Tab in Opportunities section for "X". I was thinking of adding a boolean variable to the table called X and filtering the note-history standard where x = false. The 2nd notes-history tab would be where x= true. The only problem is i cant find the standard note-history form ( i need to add a checkbox bound to X in that form). The user can tick X when creating the note-history for X otherwise X will be left unticked. Can anyone let me know the family name and form name of the default notes-history form?
