error message on a menu item

i added a form to a menu item when i click the menu item i get this message:
Form had an error:
Cannot find or access the subject required by the Personal:OTMVehicles plugin using the option you selected. Contact your system administrator. 0x1 @0: Form Personal:OTMVehicles
i have added other customized forms to be called from the menu and there was no problem. This one has me stymied.
Any suggestions?

This is because you're trying to directly invoke a data form. A data form needs context (for example, to know which row to bind to, or if it is for a new record). Instead, you'll need to create a script to trigger from the menu which uses ShowViewForRecord so you can give it context (by passing it an ID for the record to bind to, or a blank ID so it knows it is for a new record). thanks Ryan,
I decided that a managed form would better suit my needs in this case. There is no need to pull up information from the table only to insert information to the table and i can write a sql script to do the purpose i need.
