Problems with DialogService

Hello everyone,
I have faced many problems with Dialogservice, when I try to use it in a VS form instead of quickform it doesn't work presenting a blank Dialog without any objects.
A good example of this is the form AddEditAddress.ascx. I'm trying to call it from Insertcontact.aspx but no eventuate at all.
Follow the routine that I am using to call it
protected void btnAddress_Click (object sender, and ImageClickEventArgs)

if (DialogService! = null)
(
DialogService.SetSpecs (0, 0, 400, 600, "AddEditAddress", "Add and Edit Address", true);
DialogService.EntityType = typeof (Sage.Entity.Interfaces.IAddress);
DialogService.ShowDialog ();
)
