What is the best method of adding an Icon (much like the Save, Send email, Help icons on the Insert ticket page) into that top blue bar?

Do I really need to create a custom SmartPart if all I want to do, is have an Icon up there that when pressed goes to a certain URL ?

No, just click on anywhere on the form (not on one of the controls on the form) click the Toolbar ellipsis on the right hand side, and add you button there. You can specifiy whether its left/center or right and what code it should run on clicking..
